Our office participates with most major insurance providers in the region. However, due to the variances in insurance contracts, please contact our office to verify network status and provider participation.
If we participate with your plan, we will file the claim for you and wait for payment. If we do not participate in your plan, we will file the claim as an out of network provider, which may be at a higher cost to you. Payment will be required on the day services are rendered and can be made by cash, check, or credit card. Proof of insurance, photo ID, and co-payment or co-insurance are requirements for not only the first visit, but also every subsequent visit thereafter.
Insurance guidelines can be confusing; often referrals are required before office visits, labs, physical therapists and similar. We encourage you to make yourself familiar with your plan’s regulations, for management of medical reimbursement is ultimately your responsibility.
