Our office participates with most major insurance providers in the region. However, due to the variances in insurance contracts, please contact our office to verify network status and provider participation.
Payment will be required on the day services are rendered and can be made by check or credit card. Proof of insurance, photo ID, and co-payment or co-insurance are requirements for not only the first visit, but also every subsequent visit thereafter.
Our providers require a doctor to doctor referral for your initial visit to include imaging records, recent labs, physician consult notes, and progress notes. This referral and accompanying documentation will help our providers better assist you with your particular case and is not the same as an insurance required referral.

Dear Wellmed Patients:
It has been our honor to care for you in our neurology practice and infusion center. Unfortunately, we are writing to notify you that we are terminating our participation in the WellMed Networks, Inc/PIPA.
This letter is to notify you that we will continue to provide treatment and access to provider and infusion services for the next 30 days to complete your current plan of care and allow you time to secure care from another provider. Thereafter, we will cease performing any provider or infusion services for any WellMed patients.
We encourage you to refer to your WellMed provider directory to locate another neurology provider and infusion center. We are also available to assist you in finding alternative providers and infusion locations.
Upon a signed patient authorization, we will transfer your medical records to your new provider.
We are here to assist you during this transition so please do not hesitate to contact our office if you have any questions.
